Financial Disclosure for PHS-Funded Research (Form 500)

In accordance with the University of California policy, this form is to be completed by the principal investigator and those project personnel designated by the principal investigator as an "investigator" as defined by the 2011 PHS regulations to disclose their personal significant financial interests and those of their spouse/registered domestic partner and/or dependent children related to their institutional responsibilities. Residual Account Request Form

The Residual Account Request Form, in conformance with Residual Accounts Policy Number 950-01-004, allows a faculty member to request a residual account that can be used to direct-charge a portion of their X base salary to a grant, and in return, receive an allocation equivalent to the amount deposited to the residual account. The total amount of the X charged to the grant cannot exceed 85% of the X base salary. The gross salary will be immediately allocated. However, the benefits portion will be allocated at the end of the 6-month period. At the end of every 6-month period (or the end of the exchange period), the total amount allocated to my account will be adjusted to the amount actually charged to the grant.
